Wheels-up C-17 crash caused by pilot error

Pilots of a C-17 Globemaster failed to lower the transport’s landing gear, forcing them to make a crash landing at Bagram Airfield in Afghanistan, an Air Mobility Command investigation concluded.
None of the six onboard were injured; the repair bill for the $200 million aircraft, however, totaled $19 million.
Flying the Globemaster were aircraft commander Capt. Anthony J. Mione and co-pilot 1st Lt. Chad M. Dugie. Also in the cockpit: a second co-pilot, 1st Lt. James A. Linnehan, sitting behind Mione; and an airman riding as a passenger sitting behind Dugie. Loadmasters Staff Sgt. Matthew J. Conn and Airman 1st Class Kylor R. Eutsler were below in the plane’s cargo bay.
The aircrew was assigned to the 16th Airlift Squadron and 437th Airlift Wing at Charleston Air Force Base, S.C. The pilots have been grounded pending a command review of the accident investigation report, an AMC spokesman said.
Rumors that crew members hadn’t lowered the landing gear have circulated since the Jan. 30 crash after photographs from inside the plane’s cockpit showed the landing gear controls in the up position.
Mione and Dugie knew each other well, having flown 34 sorties together since deploying to the Persian Gulf, the report said. Minoe had logged 826 hours in C-17s and 751 hours in C-21A executive jets. Dugie had just 149 hours in C-17s, logging 96 of those hours in the previous three months. Linnehan was only slightly more experienced, with 248 C-17 hours.
While the pilots were distracted by a series of minor problems as they approached Bagram in the dark, the aircrew could have avoided the crash by following checklist procedures, a basic Air Force rule.
“Had they lowered the gear, the mishap would not have occurred,” concluded Col. Richard D. Anderson, accident investigation board president.
The automated “ground proximately warning system” that would have instructed the crew to lower the wheels was apparently accidentally turned off, said Anderson, who is qualified as a C-17 instructor pilot and serves as vice commander of the 621st Contingency Response Wing at McGuire Air Force Base, N.J.
The crash marked the second time in fewer than three years that the crew of a large Air Force jet forgot to lower the landing gear before skidding down a runway.
In May 2006, a B-1B Lancer bomber touched down wheels up on a runway at Diego Garcia in the Indian Ocean. The crew had turned off a landing gear warning system before touching down.
Normal flight until landing
As the C-17 neared Bagram, the flight was uneventful, the report said. The jet left Kuwait City International Airport about 6:30 p.m. with 21,000 pounds of cargo and one passenger.
For most of the flight, co-pilot Dugie flew the plane. Mione, a new instructor pilot, assisted him.
When the pilots made radio contact with Bagram air traffic controllers, the crew learned the airfield’s approach radar was not working and they would need to land using visual flight rules. The requirement meant they needed to focus on their plane’s speed and altitude and to watch for other aircraft.
To help spot mountain ridges surrounding Bagram and other aircraft, the pilots put on night-vision goggles.
The crew also went through the descent checklist and approach checklist.
About 10 miles from Bagram and flying about 250 mph, the crew extended wing slats and lowered flaps to slow down the plane, the report said.
As the plane was about three miles out, the pilots removed their night-vision goggles and aircraft commander Mione radioed “short final” to prompt the control tower for clearance to land. There was no response from the tower.
The crew continued to gradually lower the flaps, slowing the jet.
With 28 seconds left until landing, Mione took control of the jet and radioed the tower, “short final.”
The tower controllers answered this time, clearing the C-17 to land. The controllers failed to make the required reminder call — “Check wheels down.”
As the plane passed below 300 feet, Dugie didn’t announce the required alert “300 feet.”
Now, the plane was also flying at 172 mph, 42 mph faster than approach rules called for.
If Mione had followed correct procedures, he would have aborted the landing because of the high speed and made a second approach, the report said.
Instead, Mione continued to descend. The three pilots didn’t realize they missed the “before landing checklist.”
With the landing gear still up, the plane’s ground warning system should have sounded out “too low gear.” The alarm didn’t activate because the pilots accidentally turned off the system, Anderson concluded.
Mione and Dugie claimed the ground warning system must have malfunctioned, but there was no evidence the system wasn’t working, the report said.
As the jet lined up on the runway centerline, the plane touched down at 150 mph, 21 mph too fast.
With the landing gear nestled inside the wheel wells, the jet’s aluminum alloy belly ground into the concrete runway. The plane slid in nearly a straight line for 4,528 feet before coming to a stop. A fire broke out on the plane’s rear left side but was extinguished by firefighters minutes later.
Moving the grounded jet took two days and the efforts of more than 200 people and a 120-ton crane. The team moved the jet by using the crane and airbags to lift the plane high enough to lower the landing gear.

Mothers day is celebrated across the globe. In India, like in the US, it is observed every year on the second Sunday of May and hence falls on May 10th in 2009.
History of Mothers day stretches back to ancient Greek era where Rhea, mother of many Greek mythology deities was honored. Ancient Romans are also believed to have celebrated this day during the spring festival to honor Cybele, a mother goddess. In recent history, Christians began to celebrate Mothering Sunday in England during the fourth week of Lent as a tribute to mothers. Celebrations began with a church service to honor Virgin Mary and flowers and gifts were given by children to their mothers as a token of love.
Mothers day in the US started with Julia Ward Howe who started observing this day on June 2nd around 1870 to promote pacifism and disarmament by women. In those days, it was celebrated by women’s peace groups and a common practice was meetings for mothers whose sons had died fighting the American Civil War. Mothers day is believed to have been first celebrated in Albion, Michigan on May 13th, 1877. Folklore has it that Juliet Calhoun Blakeley stepped in to support and completes the sermon of an emotionally distraught Rev. Myron Daughterty, whose sons were publicly embarrassed by a local anti-temperance group. Blakeley’s sons were extremely moved by this gesture that they decided to pay tribute to her each year.
But the day gained wider recognition after 1907, when young Anna Marie Jarvis started celebrating Mothers day in Grafton, West Virginia, as a private ceremony in memory of her late mother. In the beginning, young Anna sent carnations , her mother’s favorite flowers to the church service. Later, she went on to promote this day to a national and finally an international holiday. On May 14th 1914 Woodrow Wilson, the then US President signed an official declaration stating the second Sunday of May every year as Mothers day.
Today, Mothers day is celebrated all across the world with children sending gifts, cards and lovely flowers to mothers to express their deepest love and gratitude.

PUC Results On May 9

Here’s some good news for those waiting with bated breath for the II Pre-University results. The Department of Pre-University Education is all set to declare the results on May 9 on the Internet. The results will reach the colleges a day later, sources in the department said. The evaluation was completed nearly 10 days ago but the department is waiting for the Common Entrance Test, being held on May 6, 7 and 8, to be completed. “We did not want students to be stressed out because of the results and do not want to distract them from their exams. CET is a very important exam for most students who are from the science stream,” a PU department official told The Hindu recently. As many as 6.2 lakh students appeared for the examination, which was held from March 13 to March 26 in 856 centres across the State. The number of students had increased by nearly 50,000 when compared with 2008. Of this year’s applicants 1,72,676 lakh are from the science stream, and 3,10,584 lakh and 1,36,859 lakh are from the Arts and Commerce streams respectively. Passenger car market leader Maruti Suzuki will launch its sixth global car, Ritz, in May. The mass market car will be launched in the popular small car segment with a 1.2 litre KB series petrol and 1.3 litre multijet diesel engine simultaneously.
The Ritz will be built on Maruti’s successful Splash platform and will be priced between Rs 4 lakh and Rs 5 lakh, which is just below the Swift’s price tag. It will be shorter than the Swift (by 30 mm), but will be spacious due to high roof structure. The Ritz will be positioned between the A-star and the Swift, and will counter Hyundai’s i10 and i20, Skoda’s Fabia, Ford’s Fusion, Fiat’s Palio and Tata Motors’ Indica Vista. This car is Maruti’s ninth small car that will consolidate its position in the crowded market. The suspension of the vehicle with a ground clearance of 170 mm has been tuned for Indian road conditions.
Dubbed as a multi-purpose vehicle, it will cater to small urban families and sport BS-IV engines and a 5-speed manual gearbox. The Maruti Ritz will be available with two different engine options: a brand new K-series 1.2 litre, four-cylinder, 85PS Petrol engine and the super-successful 1.3 litre, 16-valve DDiS, 75PS Diesel engine.
The Ritz, known as Splash in Europe and other developed markets, will coexist with the ageing WagonR platform, just as the newly-launched A-Star is thriving with the Alto.

A Maruti spokesman confirmed that the car will be launched sometime in the first half of the year. “We have not decided the date, but it will be launched as per schedule,” he said. According to dealers, the Ritz will be launched prior to Honda’s small car, Jazz, which is slated to hit the roads in June and Fiat’s premium hatchback, Grande Punto. The Ritz has cleared all the homologation tests and has been approved by the International Centre of Automotive Technology (iCAT) at Manesar in Haryana last week for launch in the Indian market.

Public scrutiny blunted bank stress tests

Treasury Secretary Timothy Geithner said in February that putting the nation's biggest banks through "stress tests" was vital to getting the financial system back on solid ground.
But with the results set to be released Thursday, critics say regulators seem so intent on avoiding statements that might undermine confidence in the banks that they risk eroding trust in the stress tests themselves.
Regulators say none of the 19 banks will be allowed to fold. That rules out any official statements that might scare investors. As a result, critics say the results won't likely provide the specific analysis and discipline banks need to lend money and nourish an eventual economic rebound.
The public spotlight on the tests "negates the whole point" of stress testing, because regulators know tough action could imperil the banks, said Jaidev Iyer, a former risk management chief at Citigroup Inc. who now works at a nonprofit involved in bank risk analysis.
The tests of the 19 largest financial firms are at the center of the Obama administration's plan to stabilize the financial system. The tests estimate losses banks would face in a "what-if" scenario involving a worsening recession: 10.3 percent unemployment and a 22 percent drop in home prices over the next two years.
So far, investors seem unfazed by reports that the tests found some banks would need to raise capital to absorb possible future losses. A handful of banks among the 19, including Bank of America Corp., Citigroup Inc. and Wells Fargo & Co., would need more capital based on initial findings, sources have told The Associated Press.
The Wall Street Journal reported early Wednesday that Bank of America's capital shortfall stands at about $35 billion, which would make Charlotte, N.C.-based BofA potentially the bank with the largest capital needs.
Regulators and internal auditors routinely use stress tests to manage bank risk. The tests, typically done in private, help guide investments and ensure the banks' stability. Normally, regulators disclose their evaluations and remedies with banks behind closed doors. By contrast, critics say, the Fed's approach seems designed for public consumption.
Open discussion of the stress tests, from the White House on down, has made it hard for regulators to be as candid as they'd like, said Bradley Sabel, a veteran bank supervisor with the Federal Reserve Bank of New York now at the law firm Shearman & Sterling.
"I think there's an awful lot of value in keeping confidential the discussions between banks and examiners," he said.
Fear of igniting a market panic means regulators aren't likely to force banks to make major changes in their operations and investments, Sabel said.
The administration has repeatedly called attention to the stress tests. For weeks, officials have brushed off questions about the health of the banking system by mentioning the forthcoming test results.
Asked last month about a Treasury program to buy banks' troubled assets, White House adviser David Axelrod said, "Let's see what happens once the stress tests are done and the capital needs of banks are determined."
Simon Johnson, a former chief economist with the International Monetary Fund now at the Massachusetts Institute of Technology's Sloan School of Business, said the tests may have served the administration's political needs.
"The stress test was a clever stalling action from a tactical point of view," Johnson said. "They wanted to wait until the economy showed signs of bottoming out. Now, everyone's more relaxed, and they can go easier on the banks."
Asked about the tests, a senior government official familiar with the process said they were designed to illuminate the health of the banking system. The results will be clear and detailed enough for investors to make informed decisions, said the official, who requested anonymity because he wasn't authorized to discuss the matter.
The difficulty of operating under public scrutiny was clear almost from the day Geithner announced the tests, said Kevin T. Jacques, a longtime Treasury employee who's now a finance professor at Baldwin-Wallace College.
"I think Treasury got backed into a corner," Jacques said. "It felt, 'The market is clearly aware we're doing these tests ... If we don't release the results of the stress tests, the market will think that we're hiding something.' "
Providing more information about the health of banks is a worthy goal, said William Seidman, who ran the Federal Deposit Insurance Corp. during the savings-and-loan crisis. But he said the best way to do so would be to tailor the tests to each firm. Among the 19 firms being stress-tested are an insurer, an auto finance giant and banks with diverse business models.
Applying the same scenarios to 19 firms makes little sense, Iyer agreed. A"one-size-fits-all approach" doesn't take account of the strengths and weaknesses of each bank's assets.
"I am very skeptical that we will learn much about the true conditions of these banks," he said.
Billionaire investor Warren Buffet made a similar point over the weekend. He said the stress tests focused on banks' debt — not on whether their operations were basically strong.
Federal Reserve Chairman Ben Bernanke told lawmakers Tuesday that the tests will help banks develop plans to raise their capital buffers if necessary. The extra capital would ensure the banks could keep lending even if the recession worsened.
A functional financial system will be crucial to any economic rebound. Until banks can return to normal lending, it will be hard for companies to expand. And it will be tough for consumers to make the purchases that would spark a recovery.
Yet there's no guarantee that forcing banks to boost their capital reserves will have the desired result, Seidman said. He said the government should take control of banks that might fail and clean up their balance sheets by seizing assets that have lost value or can't be sold.
Iyer said he worries the tests have become too tangled in fears of political or economic aftershocks to do much good.
"I'm a little concerned that somewhere in there, we've lost complete sight of the meaning of this exercise," he said.

EXCLUSIVE-US watchdogs want bar raised in failed bank reviews

So many U.S. banks are failing that three government bank watchdogs want the law changed to force a review only when a bank failure costs the federal insurance fund $300 million or more. The inspectors general, or IGs, of the U.S. Treasury Department, the Federal Reserve and the Federal Deposit Insurance Corp., citing a heavy workload of "material loss reviews" of failed banks, want Congress to change the law that triggers a review, according to a letter obtained by Reuters on Monday.
Currently a review must be done if a federally supervised bank's failure costs the federal Deposit Insurance Fund more than $25 million, the letter said.
If the workload is not reduced, the watchdogs' oversight of other, urgent government programs to stabilize the financial system and revive the economy could suffer, said the letter, which was sent to Representative Barney Frank, chairman of the House Financial Services Committee. The request is expected to be discussed on Tuesday at a hearing before the oversight subcommittee of the U.S. House of Representatives Financial Services Committee.
In the first four months of 2009, 32 U.S. banks failed, compared with 25 in all of 2008, and just three in 2007. There were no bank failures in 2006 and 2005.
On Friday, in the biggest bank failure of the year so far, regulators shut down Silverton Bank, in Atlanta, Georgia, at a cost to the FDIC's bank deposit insurance fund of $1.3 billion.
In a material loss review, an inspector general explores why a failed bank's problems resulted in a hit to the federal bank insurance fund, and recommends how to prevent such a loss in the future....
"We are writing to request that the Congress consider increasing the threshold" to reflect the increased size and number of bank failures, the watchdogs wrote in the letter.

Daisy Bopanna

Daisy Bopanna (Kodava Kannada : ಡೈಸಿ ಬೋಪಣ್ಣ ) is an Indian actress in Bollywood and Kannada films.

Biography
She did her schooling from Aurobindo School, pre-university at Kumarans College and graduated with a Bachelor of Fine Arts degree. She starred in more than 150 ads before getting a big break in Bollywood with the Priyadarshan movie Garam Masala. At present she lives in Mumbai. Recently she has appeared in the Kannada movie Gaalipata. Daisy Bopanna's family hails from Coorg/Kodagu, known as Kodavas / Coorgs. Daisy born and raised in Coorg did her schooling in Aurobindo School & has graduated with a bachelor's degree in Fine Arts.
Daisy Bopanna made her cinematic debut in a de-glamourised role in the Kannada film Bimba (film), a film directed by Kavitha Lankesh which explored the exploitation of child artistes in the film industry. But her later work in a movie Bhagwan with Darshanearned her the tag of “spicy Daisy”. The actor, however, is rather surprised on her moniker. “I have never done glamorous roles for the heck of it. In the past, I have refused lot of roles where I was required to wear skimpy clothes,” says Daisy.
Daisy made her Bollywood debut in Priyadarshan’s Garam Masala (film). Her first scene required her to kiss John Abraham. In Mumbai, Daisy has just shifted to a new house and is busy realising her Bollywood dreams. She is all set to star in a Hindi romantic comedy and has recently finished with the photo shoot. Another Telugu film Hrudayam directed by Jai Akash is also on. The actor had earlier done two Telugu films Chanti opposite Ravi Teja Sharma and Relax. Sweet Heart Akash, Daisy Bopanna

Directed by Akash and produced by SJ Ratnavat on the banner of Devi Movies, 'Sweet Heart' with Akash himself playing the hero and Daisy Bopanna as heroine had completed shooting and other post-production formalities recently. The first copy of the film is also ready now. Addressing the media, Akash says, 'We watched the first copy and all of us felt happy for bringing out a nice film. I was able mould the story that was in my mind exactly on the celluloid with the complete cooperation from the producers. Even we did not compromise in post-production activity and finished it in an efficient way, though we took a little more time to complete it with a view to give a quality film. The audio had already proved a hit and we are sure that the film would also be a big hit.' Music director Nandanraj says, 'Everyone eagerly looked how would be the debut direction of Akash. Once we completed the re-recording, everyone watched the film and felt very happy. Even I personally felt thriller. He did a very nice job.' Producer says, 'The film came out well. Especially the movie would definitely entertain the youth. Though this is the debut venture, Akash brilliantly brought out the movie. In fact, we started the film as a low-budget movie. However, we did not compromise as the film is coming out well the excellent quality and increased the budget. The film has everything for everyone.'

Videocon to start DTH services on May 1

Diversified business house Videocon Group is making an ambitious foray into the direct-to-home services with plans to garner one crore subscribers in the next six years.
The group, which made an announcement of Rs 1,000 crore for the DTH venture earlier, made a soft launch of its 'Videocon D2H' services last evening in Macau in front of its 2,500 dealers, who where specially flown in there.
“The full commercial roll-out will start from May 1. In the first year, we are looking at 15 lakh subscribers and add similar number every year. The target is to have one crore subscribers in six years,'' Videocon Group Chairman Mr Venugopal Dhoot told P TI. He said the company is set for a mega roll out of its DTH services as the 2,500 dealers will have around 15,000 showroom across the country.
“That gives us an advantage. Besides, we will also be offering TVs with in-built set-top boxes (STBs), which no one in the market offers,'' Dhoot said. He said over 40 engineers of the group spent nearly two years in developing the TV sets with in-built STBs. The DTH services will leverage on Videocon's brand equity.
“We had also unveiled a new brand logo for the group, which will be common for DTH, mobile services and the consumer durables segment,'' he said.

Bigger than BIG TV! Check it NOOOOOOOW

Videocon to launch DTH in OctoberVideocon that plans on launching its DTH service by the end of October, will be the only Direct to Home provider that will manufacture their own set top boxes and apart from that it will the next provider to offer MPEG 4 technology to its customers.
But what makes Videocons service so great that it would compete with Big, firstly unlike Big Entertainment who has been unable to give the media or the consumers a fixed launch date, Videocon seems to have planned out its execution and road map much better.
Anil Khera who was earlier in charge of Videocon's Sansui and Kelvinator is heading Videocons D2H services. Khera said, "We have taken six Ku-band transponders on SingTel's ST-1 satellite. We are investing Rs 10 billion over two years and are launching the DTH service by October-end."
Videocon will be manufacturing STBs in it factory in Aurangabad, this may be beneficial to consumers as they would be able to provide them with the STBs at a lower price point. While Big TV uses AsiaSat for its transmissions and buys STBs from manufacturers in China and Taiwan.
Although Videocon plans on launching with around 200 channels which will be close to forty less than Big, prices have always been important to the indian consumer, lets wait and watch as the Direct to home segment heats up, which will open a lot of doors to the Indian TV watcher.

Obama Speaks

President Obama said he will ask all of his department and agency heads for specific proposals for cutting their budgets at his Cabinet meeting early next week as he searches for ways to streamline government spending.

Obama, who is attending the Summit of the Americas in Trinidad this weekend, said in his weekly radio and Internet address that he would make the request for cuts Monday at a Cabinet meeting.

“In the coming weeks, I will be announcing the elimination of dozens of government programs shown to be wasteful or ineffective,” he said. “In this effort, there will be no sacred cows and no pet projects. All across America, families are making hard choices, and it’s time their government did the same.”

While discussing the need for more efficient government, Obama announced he was filling an administration position that caused him trouble on the last try. Obama said Jeffrey Zients, a CEO, management consultant and entrepreneur, will join the administration as the government’s chief performance officer and will also serve as deputy director for management of the Office of Management and Budget. He will work to streamline processes and cut costs, Obama said.

Also he said that he would soon announce the elimination of dozens of wasteful or ineffective government programs as part of a broad effort to restore fiscal accountability to the federal budget.Obama, speaking in his weekly radio address, said he would use his first full Cabinet meeting on Monday to ask department and agency heads for specific proposals for trimming their budgets.He named two new officials as part of a team of management, technology and budget experts that will drive the process of trimming the fat and waste from government spending."As surely as our future depends on building a new energy economy, controlling healthcare costs and ensuring that our kids are once again the best educated in the world, it also depends on restoring a sense of responsibility and accountability to our federal budget," Obama said."Without significant change to steer away from ever-expanding deficits and debt, we are on an unsustainable course," he added.The United States posted a record $956.8 billion budget deficit for the first half of fiscal 2009, more than three times the shortfall of a year ago, the Treasury Department reported earlier this month.Much of the deficit was caused by spending on financial and economic rescue programs aimed at propping up companies whose collapse could worsen the global recession.Obama said Cabinet officials already had begun cutting back unnecessary expenditures, including a consulting contract to create new seals and logos that cost Department of Homeland Security $3 million since 2003.The president also commended Defense Secretary Robert Gates' project to reform defense contracting procedures to eliminate what he said were hundreds of billions of dollars in wasteful spending and cost overruns."If we're to going to rebuild our economy on a solid foundation, we need to change the way we do business in Washington," Obama said. "We need to restore the American people's confidence in their government -- that it is on their side, spending their money wisely, to meet their families' needs."He named Jeffrey Zients, a management consultant and entrepreneur, to act as chief performance office with the official title of deputy director for management of the Office of Management and Budget.Zients is a director of Sirius XM Radio and served as chief executive of The Advisory Board Company.His job will be to streamline processes, cut costs and find best practices throughout the government, Obama said.He named Aneesh Chopra, the secretary of technology for the state of Virginia, to be the U.S. chief technology officer charged with promoting technological innovation.

Non-Tech Guy Appointed As CTO by Obama

“President Barack Obama has named his chief technology officer, and the appointee is not a Silicon Valley name like so many predicted. He is Aneesh Chopra. As the Secretary of Technology for the Commonwealth of Virginia, his job has been to ‘leverage technology in government reform, promote Virginia’s innovation agenda, and foster technology-related economic development with a special emphasis on entrepreneurship.’ But Chopra’s not a tech guy. Before he got his secretary job in 2005, he was a managing director at the Advisory Board Company, a public-market health care think tank, as well as an angel investor.”

Aneesh Chopra as the nation's first Chief Technology Officer. Chopra, who has effectively been doing much the same job at a state level in his role as Secretary of Technology for Governor Kaine of Virginia , will closely work with Vivek Kundra.

[The role of the CTO is to provide visionary leadership, to help a company (or in this case, a government) explore the transformative potential of new technology.]
